Short Description
The grant is designed to reduce the rates of teen pregnancy and existing disparities by replicating evidence-based teen pregnancy prevention programs to scale in at least three settings: 1) in-school middle schools, 2) in-school high schools and 3) alternative schools and/or community-based afterschool programs. Services target youth ages 12-18 in a designated service area. The program is comprised of five major components, all of which are aimed at reaching youth to achieve the goal of decreasing teen birth rates: 1) deliver evidence-based curricula, 2) identify youth-friendly organizations and providers for the purpose of linkage, 3) develop and work with Community Advisory Group (Coalition), 4) develop and work with a Youth Leadership Council (YLC), and 5) develop a strategic dissemination and communication plan.

Goal/Long Term Outcome: Reduce the rate of births to teen females (age 15-19) per 1,000 females age 15-19 in communities.
