Short Description
This special project provides outreach and education to community organizations and providers on the WIC Program, and also identifies program participation barriers and ways to address them.

(a) The Federal awarding agency must notify the public of Federal programs in the Catalog of Federal Domestic Assistance (CFDA), maintained by the General Services Administration (GSA).

(1) The CFDA, or any OMB-designated replacement, is the single, authoritative, governmentwide comprehensive source of Federal financial assistance program information produced by the executive branch of the Federal Government.

(2) The information that the Federal awarding agency must submit to GSA for approval by OMB is listed in paragraph (b) of this section. GSA must prescribe the format for the submission.

(3) The Federal awarding agency may not award Federal financial assistance without assigning it to a program that has been included in the CFDA as required in this section unless there are exigent circumstances requiring otherwise, such as timing requirements imposed by statute.

Objectives and Goals
Performance is measured by the number of eligible women and families who receive information about WIC, CSFP and FMNP programs with a goal of reaching at least 5,000 women and families each quarter and convening Community Quality Councils at each of the 15 DHS local offices at least quarterly.

Eligibility Requirements
This FY17 funding opportunity is limited to those public or private, not-for-profit community-based agencies who currently receive funding from the Illinois Department of Human Services, Division of Family and Community Services for the implementation of WIC Outreach, described herein, AND continue to meet the additional eligibility criteria below. Failure to provide the requested information as outlined in the NOFO to demonstrate these criteria are met will result in the application being removed from funding consideration.

The selected provider must be in a position to begin providing services on July 1, 2016 at 12:01 a.m.

Project Grants are posted to grants.gov and applicants must follow the procedures noted in the Request For Application. Environmental impact information is not required for this program. This program is eligible for coverage under E.O. 12372, "Intergovernmental Review of Federal Programs." An applicant should consult the office or official designated as the single point of contact in his or her State for more information on the process the State requires to be followed in applying for assistance, if the State has selected the program for review.

Application Procedures:
2 CFR 200, Uniform Administrative Requirements, Cost Principles, and Audit Requirements for Federal Awards applies to this program. Local agencies and retailers must apply to the State agency in writing. Individual participants apply for WIC benefits at an approved local agency. State agencies must submit required documentation to FNS.

Post Assistance Requirements
100% of quarterly reports will be submitted by the 15th of October, January, April and July documenting the goal of:
Reaching at least 5,000 women and families each quarter
Convening Community Quality Councils at each of the 15 DHS local offices at least quarterly
100% of monthly expenditure documentation reports will be completed and submitted by DHS by the 15th of the following month.
