Short Description
The Illinois Department of Public Health’s Immunization Section, is requesting applications from existing qualified local health departments, trained and certified by IDPH to assist IDPH in conducting inspections of local Vaccines For Children (VFC) enrolled-providers to ensure compliance with VFC program requirements. Agencies will assess provider compliance with CDC eligibility and vaccine storage and handling requirements and will provide education and feedback to provider staff to improve coverage levels and immunization practices. Agencies will collaborate with the Department to ensure that all active VFC providers have a site visit every other year.

Funding for the program is approved by the Office of Management and Budget (OMB) and allocated through the Centers for Medicare & Medicaid Services (CMS) to the Centers for Disease Control and Prevention (CDC). The CDC then partners with each state to provide funding for Vaccines for Children Compliance coverage in Illinois.

12 health Departments listed previously are the only allowed applicants for this grant as part of a 5 year grant/project term with the Centers for Disease Control (CDC), this is the final year of the 5 year term that identified the specific grantees.

Monthly Reimbursements and progress Reports are required, and must be received by the IDPH no later than 5pm of the 20th of the month following.
