Short Description
As the COVID-19 Corona virus crisis continues, the profound need for enhanced technological resources for Care Coordination Units to sustain their operations is present. As the workforce deploys various remote work strategies, enhanced technology will assist to further ensure the health, safety and welfare of vulnerable older adults residing in Illinois.

The intent of this award is to provide technological enhancements to the CCU program for participant services provided through the CCU which support the capability for electronic signatures and ease of use when interfacing with IDoA's current reporting and billings systems.

Applicants of this funding opportunity must demonstrate how the funds will be used to enhance the CCU's technology tools to increase its capacity to competently and effectively complete more effective and efficient Care Coordinator activities within a participant's home.
Eligibility Requirements
Only CCU agencies currently contracted with IDoA may apply for this award.

Application and Award Processing
IDoA estimates forty-four (44) awards to be administered to contracted CCU's in all Planning and Service areas throughout the state. Application and submission information available in the NOFO supplemental section D.
Assistance Consideration
The total award per CCU is up to $25,000.00. The following factors will be taken into consideration for the final amounts awarded to each grantee:
Number of participant assessments completed during FY19.
Number of FTE CCU supervisors, FTE Certified Care Coordinators, and FTE support staff who access IDoA technology at the time of the grant.
Number of separate offices locations.
Post Assistance Requirements
Grantees shall be required to submit financial and programmatic reports, minimally, for the following reporting periods:
1. July 31, 2020
2. August 1, 2020 - January 31, 2021
3. February 1, 2021 - June 30, 2021

The Department reserves the right to award to all, some, or none of the applicant entities for this funding opportunity.
This agreement is contingent upon and subject to the availability of sufficient funds.
