Short Description
In response to Funding Opportunity Announcement (FOA) No. TI-18-007, IDHS SUPR submitted an application to Substance Abuse and Mental Health Services Administration (SAMHSA) for the Illinois Rural and Small Urban Hospital-Based SBIRT Project. Participants were named in the application and will provide Screening, Brief Intervention, and Referral to Treatment (SBIRT) services as specified in the application.

Objectives and Goals
The purpose of this program is to implement screening, brief intervention and referral to treatment services for adolescents and adults in primary care and community health settings (e.g., health centers, hospital systems, etc.) for substance misuse and substance use disorders (SUD). This program is designed to expand/enhance the state continuum of care for SUD services and reduce alcohol and other drug (AOD) consumption and its negative health impact, increase abstinence, reduce costly health care utilization, and promote sustainability and the integration of behavioral health and primary care services through policy changes that increase treatment access in generalist and specialist practice. Application and Award Processing
Applicants must apply for or update their DUNS number ( ). This must be done yearly.
Apply for or update their SAM registration and receive a SAM cage code. ( This must be done yearly.
Be registered and in good standing with the Illinois Secretary of State. ( ) (This is not required of governmental entities and schools.)
Register with the GATA/CSFA system at . The system will automatically check your registrations with the above mentioned systems. It will also check to see if your entity is on the federal excluded parties list and if your entity is on the State of Illinois Stop Pay/Compliance list. If your entity is not up-to-date on the registrations, or is listed on the federal list or Illinois Stop Pay/Compliance list, your application will not be accepted until these situations are resolved.
Applicants must complete a Uniform Grant Application, Uniform Budget Template, the Administrative and Fiscal Risk Assessment (Internal Control Questionnaire-ICQ), and Programmatic Risk Assessment.
Assistance Consideration
There are no requirements for match or cost sharing.
Post Assistance Requirements
In accordance with the regulatory requirements provided at 45 CFR 75.113 and Appendix XII to 45 CFR Part 75, recipients that have currently active Federal grants, cooperative agreements, and procurement contracts with cumulative total value greater than $10,000,000 must report and maintain information in the System for Award Management (SAM) about civil, criminal, and administrative proceedings in connection with the award or performance of a Federal award that reached final disposition within the most recent five-year period. The recipient must also make semiannual disclosures regarding such proceedings. Proceedings information will be made publicly available in the designated integrity and performance system (currently the Federal Awardee Performance and Integrity Information System (FAPIIS)).
