Short Description
The purpose of the Illinois Department of Public Health Elimination of Disparities Program is for grantees to create targeted interventions to address areas of low, lagging and hesitancy related to vaccination coverage in children, adolescents and adults.

Grantees will implement prioritized achievable and affordable activities to reduce and eliminate disparities. Grantees will prepare a prioritized list of activities with evidence-based justifications, and implement three evidence based interventions with 2-3 contractors.

Funding for the program is approved by the Office of Management and Budget (OMB) and allocated through the Centers for Medicare & Medicaid Services (CMS) to the centers for Disease Control and Prevention (CDC). The CDC partners with each state to provide funding for improving vaccine coverage in Illinois.
