Short Description
The purpose of this grant is to support the development, implementation, and evaluation of initiatives to prevent healthcare associated infections (HAI) and antimicrobial resistance (AR) in Illinois, in line with national HAI/AR prevention targets. The grantee will analyze and apply relevant HAI/AR data to prioritize healthcare facilities and providers for participation in quality improvement efforts.

Objectives and Goals
The purpose of this grant is to support the development, implementation, and evaluation of initiatives to prevent healthcare associated infections (HAI) and antimicrobial resistance (AR) in Illinois, in line with national HAI/AR prevention targets. The grantee will analyze and apply relevant HAI/AR data to prioritize healthcare facilities and providers for participation in quality improvement efforts. Activities may entail occasional local travel to partner meetings or IDPH offices in downtown Chicago.

Funding may not be used for research, clinical care, and in general not for furniture or equipment in accordance with federal guidelines.
Eligibility Requirements
This is a non-competitive renewal of a grant that was originally awarded as a competitive grant.

The grantee must have a minimum of 5 years of experience in administering grants (federal, state, or private). The grantee must also have at least 5 years of experience in regional or statewide quality improvement programming across a variety of healthcare settings (inpatient, post-acute care, outpatient) specific to prevention/control of healthcare associated infections and antimicrobial stewardship. Sub-awards will require prior written approval from the program.

In addition to quarterly performance reports, provide ad hoc reports on performance metrics as requested by CDC or the Department’s Division of Patient Safety and Quality leadership.
