Short Description
This grant will allow the grantee to provide necessary treatment adherence and outreach services to persons living with HIV/AIDS in Illinois in order to access services through the Ryan White Part B program.

The Grantee will conduct outreach activities to target providers and service professionals that have contact with individuals who may or may not know their HIV status and are not in care, have not returned for treatment services, or do not adhere to treatment plans.

Outreach activities will be conducted to link persons with HIV/AIDS into care. Linking persons living with HIV/AIDS to Ryan White Part B regions is critical for access to the broad range of services provided by the agencies, including case management, outpatient/ambulatory health, oral health care, mental health, substance abuse, and supportive services.

Eligibility Requirements
Applicants should have prior direct experience working with HIV-positive individuals, or have had a previous grant from the Illinois Department of Public Health HIV/AIDS Section.
